Colorado (CO) For the 1st of the month, enroll before the 15th of the prior month. 
Connecticut (CT) For the 1st of the month, enroll before the 15th of the prior month.
District of Columbia (DC) For the 1st of the month, enroll before the 15th of the prior month.
Idaho (ID) For the 1st of the month,needs to be 15th of prior month. 
Kentucky (KY) For the 1st of the month, enroll before the 15th of the prior month.
Maine (ME) For the 1st of the month, enroll before the 15th of the prior month.
Maryland (MD)

For the 1st of the month, enroll by the end of the month. 

after January 1st of the month- needs to be 15th of prior month

Massachusetts (MA)

For the 1st of the month, enroll before the 15th of the prior month.

after January 1st of the month- needs to be 23th of prior month

Minnesota (MN) For the 1st of the month, enroll before the 15th of the prior month.
New Jersey (NJ) For the 1st of the month, enroll by the end of the month.
New Mexico (NM) for a 1st of the month- needs to be 15th of prior month
New York (NY) For the 1st of the month, enroll before the 15th of the prior month. 
Pennsylvania (PA) for a 1st of the month- needs to be 15th of prior month
Rhode Island (RI) For the 1st of the month, enroll before the 15th of the prior month.
Vermont (VT) For the 1st of the month, enroll before the 15th of the prior month
Virginia (VA) For the 1st of the month, enroll before the 15th of the prior month
